Conditions
At David Schumer MD and Modeline Jules NP Family Clinic, we specialize in treating a wide array of medical conditions. Our experienced primary care team, led by Dr. David Schumer and Nurse Practitioner Modeline Jules, is dedicated to providing personalized care tailored to your specific needs.
From common ailments to complex health issues, we offer comprehensive treatment options and compassionate support to help you achieve optimal health and well-being.
Acute Care
Our acute care services encompass the diagnosis and treatment of sudden and severe medical conditions that require immediate attention to prevent further complications. These conditions include a patient experiencing a traumatic injury, new illness, or aggravation of their chronic condition. When it comes to acute care, we take the time to sit down with and listen to our patients about their symptoms before devising a treatment plan that addresses all of their concerns.
In addition to our acute care services, we offer comprehensive primary care for a wide range of other common conditions, including:
Anxiety & Depression
Anxiety manifests as excessive worry, fear, and tension, while depression manifests in persistent sadness, loss of interest in pleasurable activities, and feelings of hopelessness. Both conditions can significantly impair daily functioning and quality of life. Mental health conditions like these can be caused by a mix of different things, including genetics, chemical imbalances, traumatic experiences, and environmental stress.
Getting a handle on anxiety and depression takes a team effort. In addition to medication management, we also provide our patients with the tools and resources they need to seek out therapy, make positive lifestyle changes, and find support networks in the area.
Diabetes
Diabetes mellitus is a chronic metabolic disorder characterized by elevated blood sugar levels (hyperglycemia) resulting from insulin deficiency, insulin resistance, or both. Type 1 diabetes is an autoimmune condition in which the immune system attacks the insulin-producing beta cells of the pancreas. Type 2 diabetes typically develops in adulthood and is associated with insulin resistance, obesity, and unhealthy dietary habits.
Uncontrolled diabetes can lead to serious health concerns, including cardiovascular disease, kidney failure, neuropathy, and a range of more severe complications. At our practice, we work with patients on an individual level to provide them with a chronic disease management plan that best suits their needs.
COPD
Chronic obstructive pulmonary disease (COPD) is a progressive lung disease characterized by airflow limitation and respiratory symptoms like chronic cough and shortness of breath. The two main forms of COPD are chronic bronchitis and emphysema, often coexisting in affected individuals. COPD is typically caused by cigarette smoking, although other factors such as genetics, exposure to air pollution or chemicals at work, and chronic respiratory infections may also contribute to its development.
As with diabetes, those who do not control their COPD are at serious risk of developing additional health issues. We are happy to develop a comprehensive chronic disease management plan to help our patients live a happier, healthier life.
Hyperlipidemia (High Cholesterol)
Hyperlipidemia refers to high levels of fats (lipids) in the blood, including cholesterol and triglycerides. It is a major risk factor for cardiovascular diseases like heart attack and stroke.
Our team may recommend lifestyle modifications to manage your high cholesterol, such as dietary changes, regular exercise, and weight management. We may also prescribe medications like statins as part of your treatment. Our providers are dedicated to providing comprehensive chronic disease management plans for our patients who suffer from hyperlipidemia.
Hypertension
Hypertension, or high blood pressure, is a common cardiovascular condition characterized by elevated arterial pressure. It is a major risk factor for heart disease, stroke, kidney failure, and other vascular complications. Hypertension often develops silently over many years and may not cause noticeable symptoms until it reaches a critical stage.
For our patients that suffer from chronic hypertension, or high blood pressure that does not ease with lifestyle changes, we offer comprehensive chronic disease management plans.
Obesity
Obesity is a prevalent health issue characterized by an excessive amount of body fat. It poses a significant risk to your heart, bones, and organs, potentially leading to diseases such as diabetes, heart disease, and various types of cancer. Obesity often progresses gradually and may not present noticeable symptoms until it becomes severe.
For our patients struggling with obesity or excessive weight that is difficult to manage with lifestyle modifications alone, we provide comprehensive chronic disease management plans. Our treatment options may include prescribing medications and personalized nutritional guidance.
Geriatric Care
Geriatrics is a specialized field of medicine that focuses on the unique health concerns of older adults, typically aged 65 and older. With aging comes an increased risk of chronic diseases, cognitive decline, functional impairments, and social isolation.
Geriatric care addresses these challenges through comprehensive assessments, personalized treatment plans, preventive treatment plans, medication management, and consistent support for caregivers. The goal is to enhance the health, independence, and quality of life for older adults, ensuring their comfort for years to come.
Geriatric Care for Dementia
Dementia is a neurological disorder characterized by a slow decline in cognitive function, progressive memory loss, and diminished daily functioning. Alzheimer's disease is the most common and well-known type of dementia. The exact cause of dementia varies depending on the underlying condition but often involves a combination of genetic factors, abnormal protein deposits in the brain, vascular changes, and neurotransmitter imbalances.
We offer dedicated care to our patients suffering from dementia and dementia symptoms. In addition to providing support in-office, we provide our patients and their families with the tools and resources they need to live a healthier life.
